SCHEDULE APPOINTMENTS
As part of our goal to promote continuity of your medical care, staff will arrange routine visits that are convenient to you. We understand that your time is valuable and we will do our best to see you in a timely fashion.
Please advise our receptionist of the nature and severity of your health issues when you call to schedule appointments. Prior to scheduling any appointment, please ensure these are authorized by your health insurance carrier(s). To set up an appointment , please call our office at (562) 431 9200 during business hours as follows:
Monday – Thursday: 9:00am to 5:00pm (except holidays)Friday: 9:00am – 12:00pm (except holidays)
PHYSICAL EXAMS (PE) OR CHOLESTEROL (LIPIDS), PAPS, MAMMOGRAMS & VACCINATIONS
Yearly physical exams or cholesterol (lipids) require 10-hr fasting: Fasting means no food or drink (water is OK) for at least 10 hours prior to your lab test. Please advise staff that you are fasting, so they may schedule a morning appointment. Patients who are seen by the doctor later in the day or afternoon, and need lab work will be requested to return for “fasting labs” on a morning convenient to patient. Adult male or female having a complete physical exam will need to complete a Patient Health History Questionnaire (New Patients) or Patient Information Update form (Established Patients). Go to: Checklist and Forms for New and Established Patients. PAPs and Mammograms : Please contact your health insurance carrier to confirm eligibility. Children's sports, camp, and kindergarten physicals (forms supplied by individual schools/facilities). Vaccination Records are required for Camp/School physical, travel consultation, annual physicals and adoption physicals.
